Manage your clients’ care in one place.

VivaLynx gives home care agencies, home health providers, and PACE organizations one record for each person they serve. Appointments, medications, visits, care notes, and vitals readings live in one place. Caregivers, coordinators, clients, and authorized family members all work from the same record, so an update made once is current for everyone, enabling better communications.

The VivaLynx caregiver console in a browser beside the mobile app: a list of care recipients showing each one's last check-in, mood and next appointment, and one client's appointments, health and tasks on a phone.

Everything they need for the visit

Coordinators assign tasks to professional caregivers, see status and issues early, and support quality of care. Professional caregivers see the care plan and the latest notes, record the visit, and know what needs attention without calling the office. Clients and authorized family work from the same record, so one update is current for everyone.

One client's care plan: today's appointments, then check-in, medications, health readings and care notes, with the day's tasks below.
Care notes for one client, newest first, each tagged by subject and importance and signed by whoever wrote it.
Biometrics for one client: heart rate, blood oxygen, blood glucose and weight, each stamped with its time and the device the reading came from.
The care circle around one client: family caregivers and professional caregivers in one list, each showing whether their access is active.

Clearer communications and oversight

Coordinators and administrators work in a browser, not the app. They see caseload, visits, tasks to do and any readings that went out of range — with who has already responded to each one.

The console a coordinator works in. No sound.

What agencies ask first

Does this replace the clinical record we already run?

No, and it does not write into one. If your clinical team charts in Epic, Cerner or HCHB, they carry on charting there. What VivaLynx holds is the part of a client’s care that lives outside those systems: the visits, the medications as they are actually given, the readings, and the notes from whoever was last in the house. When your clinical team should see that, you add them to the care circle and they see it.

Will my staff end up documenting twice?

Not for anything VivaLynx holds. The visit a caregiver records on their phone is the same record the office reports run off, so nothing is re-keyed between the two. What it adds is the part you do not get today — family and the caregivers who are not on your payroll contributing to the same picture. That is more than time in, time out and a checklist, and it comes from the people closest to the client.

What it changes for the organization

The caregiver view and the office view are the same record. Most of what follows comes from that one fact.

  1. Fewer calls, less chasing

A caregiver who can see the care plan, the last visit and what is outstanding does not ring the office to ask. Neither does the family, because they are looking at the same record. That is time back for your schedulers, every day.

  2. Staff who are set up to do the job

Your caregivers arrive knowing what happened last time and what needs attention. Nobody writes the same note twice, and nobody is the only person who knows something. The people doing the visits are the ones who feel that first.

  3. Data you can act on, not assemble

Compliance and payroll reports run off visits your caregivers have already recorded, so the reporting is a by-product of the work rather than a second job at the end of the month. Readings that go out of range surface on the day, against limits you set per client.

  4. A client whose care holds together

Family, your caregivers and the clinicians they are under all work from one plan. The client stops repeating their own history, and the people around them stop working from four partial versions of it.
